How to Reduce A/R Days in a Skilled Nursing Facility

High A/R days are a symptom, not a cause. In skilled nursing, the days pile up in a handful of predictable places — and most of them start before a claim is ever submitted. Here is where to look.

When a skilled nursing facility's cash flow tightens, A/R days are usually the metric that raises the alarm. But the number itself does not tell you what is wrong — it tells you that something upstream is slow. Lowering it means finding where the days actually accumulate, which is rarely in the billing office alone.

What A/R Days Actually Measure

Days in A/R measures how long, on average, it takes to collect payment after care is provided. A lower number means faster collection and fewer things going wrong between service and payment. A rising number means claims are staying unpaid longer — and in skilled nursing, the reasons cluster in a few specific places.

Where the Days Pile Up

In our experience, high A/R days in a SNF trace back to a short list of upstream problems:

  • Eligibility and coverage gaps. A resident's coverage changes mid-stay and no one catches it, so claims go out against coverage that no longer exists and come back denied.
  • Assessment-driven errors. Under PDPM, the MDS sets the rate. A coding or documentation problem in the assessment surfaces later as a denial, weeks after the claim went out.
  • Slow or missing charge capture. Therapy, pharmacy, and ancillary charges that are captured late or incompletely delay the claim or force a rebill.
  • Denials worked reactively. When denials are handled only as they arrive, rather than to root cause, the same denial recurs and each one restarts the payment clock.
  • Follow-up by whoever shouts loudest. Without systematic follow-up by payer and aging bucket, claims sit in payer queues simply because no one is working them in order.

Notice how few of these live in the billing office. Most start at admission, in the assessment, or in charge capture — which is why "work the billing harder" rarely moves the number for long.

The Changes That Actually Lower A/R Days

Reducing days in A/R is mostly about moving work upstream and making follow-up systematic:

  • Verify eligibility before care generates charges, and re-verify when a resident's coverage could have changed during the stay.
  • Get the assessment right before it is submitted. An accurate MDS that supports the rate prevents the denial that would otherwise appear a quarter later.
  • Reconcile charges at triple-check, so clinical, therapy, and billing agree before the claim goes out rather than after it is denied.
  • Submit clean claims on a schedule and work clearinghouse rejections immediately, before they age.
  • Work denials to root cause, so the same error stops recurring, and appeal with documentation rather than resubmitting blindly.
  • Follow up systematically by payer and bucket, so no claim sits unworked simply because it was not the loudest.

Watch the Right Numbers

A single A/R-days figure hides more than it reveals. The facilities that keep the number low tend to watch it broken down:

  • A/R days by payer — Medicare, Medicaid, managed care, and private pay behave very differently.
  • Aging by bucket — what share sits past 90 and 120 days, and whether that share is growing.
  • Denial rate and denial reasons — the leading indicator of days about to climb.
  • Clean claim rate — how much rework is happening before claims even reach a payer.

Broken down this way, a rising A/R-days number points you to the payer or the stage that is actually slowing down, instead of leaving you to work harder everywhere at once.

The Honest Part

There is no single lever that drops A/R days overnight, and any vendor who promises a specific number without seeing your payer mix and your aging is guessing. What is reliable is the direction: when eligibility, assessment accuracy, charge capture, and follow-up each have an owner and a cadence, the days come down and stay down.
