Managing Medicaid-Pending Accounts Receivable in a Nursing Facility

Medicaid-pending is the hardest bucket in a nursing facility's aging — real care already delivered, with no confirmed payer yet. Managed loosely, it becomes bad debt. Here is how to keep it collectible.

Every nursing facility has a Medicaid-pending bucket, and it behaves unlike any other part of the aging. The resident is in the building. The care is being delivered. But the payer is not yet confirmed, the eligibility date is not yet set, and the balance grows every day the application sits unresolved.

Managed well, most of that balance converts to paid Medicaid claims once eligibility is approved. Managed loosely, it quietly ages past the point of recovery and becomes a write-off nobody chose to take.

Why Medicaid-Pending Is Different

Three things make this bucket uniquely risky:

  • The receivable exists before the payer does. You are accruing charges against an application that has not been approved.
  • It depends on things outside billing — the Medicaid application, the financial determination, and in Texas the LTCMI and its medical necessity review all have to resolve favorably.
  • It is easy to lose track of. A pending account can sit in the aging looking like any other balance, while the real action — or inaction — is happening on a state portal nobody in billing is watching.

Track Three Things Together, Not Separately

The single biggest improvement most facilities can make is to stop tracking the application, the assessment, and the receivable as three unrelated items. For a pending account they are one situation:

  • The application — where it stands with the caseworker, what documentation is outstanding, whether financial eligibility has been established.
  • The clinical side — in Texas, whether the LTCMI has been submitted and whether medical necessity has been approved, since a denial there sinks the application.
  • The receivable — the balance accruing, the expected eligibility date, and what will be billable retroactively once approval lands.

When one person or team can see all three for each pending resident, the accounts that are drifting become obvious. When they are scattered across a caseworker's queue, an MDS coordinator's portal, and a biller's aging report, drift is invisible until it is expensive.

Bill Retroactive Eligibility Promptly

When a Medicaid application is approved, eligibility is frequently backdated to an earlier date, which makes the care already delivered billable. That retroactive window is real money — but it is also time-limited by billing rules, and it requires someone to act the moment approval comes through.

Facilities lose retroactive revenue in two ways: they do not notice the approval quickly, or they notice but the claims are not prepared and submitted before timely filing closes. Both are avoidable with a pending list that is worked, not just maintained.

Keep a Real Pending List

A useful Medicaid-pending list is more than a filter on the aging report. For each resident it should show:

  • Admission date and current status (private-pay conversion, application filed, MN pending, approved).
  • The expected or established eligibility date.
  • Outstanding items — what the caseworker still needs, whether the LTCMI is clear, what is blocking approval.
  • The balance accruing and what becomes billable on approval.
  • The next action and who owns it.

Reviewed on a set cadence, that list turns Medicaid-pending from a passive bucket into a worklist. Accounts stop aging by default.

Know When a Pending Account Is Really Uncollectible

Not every pending account is recoverable. Applications get denied for financial reasons, residents transfer or pass away mid-process, and some balances genuinely cannot be collected. Part of managing this bucket well is identifying those honestly — with a documented reason — rather than leaving them to inflate the aging and disguise the accounts that are still winnable.

The goal is a pending bucket where every account is either actively progressing toward payment or has a documented reason it will not — and nothing is simply sitting.

Medicaid-pending balances piling up in your aging? We will review the bucket and tell you what is genuinely recoverable, what needs action, and what should be written off.
